I've seen a few at the Corel Centre..
We had a lot of hope for that loser... I would be seriously P.O'd if I'd've doshed out the dough for a Yashin Jersey

I drew all of it myself (except for the Leafs logo). Jersey, letters, numbers, you name it.
It's one my biggest hobbies to design NHL jerseys on my computer. Take a look at my portfolio:

What program do you use?
I use MS Paint to draw them. I draw all the letters and numbers by hand, often pixel by pixel. I have dozens of templates I use for my own use.
To resize them so that they look smoother, I use Adobe Photoshop.
